PremedGo runs an AI-powered preventive healthcare platform serving more than 1,000 users and 15 B2B clients. We joined as the sole technical hire, which in a startup means one thing: everything technical is now yours.
A live platform with real users and paying B2B clients, and no in-house technical team behind it. Infrastructure, the mobile app, the data model, the security posture, the sales tooling: all of it needed an owner, and hiring a full team was neither fast nor affordable.
On top of that, monthly bills for tools like ReTool and Webflow were doing jobs that custom-built software could do better and cheaper.
We took ownership of the entire technology stack, end to end:
Infrastructure & CI/CD
Built and managed cloud infrastructure on DigitalOcean and Coolify, with full CI/CD pipelines for staging and production.
Product fixes that mattered
Shipped critical mobile app fixes, including payment processing and multi-currency support.
Replacing expensive tools
Designed and built an admin portal, marketing website and multiple web applications in-house, replacing ReTool and Webflow and cutting recurring costs.
Data model & documentation
Redesigned the provider and services data model, migrated the records across, and documented the full system architecture so the knowledge lives with the company.
Security & operations
Managed email migrations across three platforms, set up domain emails, configured Bitwarden for credential management and rotated all production secrets. Integrated HubSpot and configured Apollo for outbound sales.
And the parts of the job that never fit in a ticket: supporting AI chatbot development, attending investor meetings, and helping recruit technical talent.
PremedGo gets a complete technology function without building a technology team: infrastructure, product, data, security and sales tooling, all owned and moving at startup speed.
This is what CTO-as-a-Service looks like in practice. Not advice from the sidelines, but ownership of the stack, with the architecture documented so it belongs to the company rather than to one person’s head.
